2. The 1st Option Home Health & Infusion team will perform a pre-discharge assessment, verify insurance, and call you back.

3. 1st Option Infusion will compound all medications and gather supplies and equipment needed, and 1st Option Home Health will coordinate the timing for the nursing visit.
